Real

Real is a seller's label for services said to be delivered by genuine accounts operated by people, and it describes the catalogue listing rather than anything verified about the accounts in your order.

The word covers different things in practice. It can mean accounts that look human, with a photo, posts and some history, even if they were created or bought for the purpose. It can mean accounts belonging to people who are paid or rewarded to follow, like or watch. It cannot mean people who found your content and chose to engage with it on their own, because an order placed on a panel is by definition not that.

Nobody in the chain verifies the label. A reseller copies the service name from its supplier, and the supplier chose the word when it listed the service. If the supplier changes its account source, the name usually stays, so the same listing called real can deliver differently from one month to the next.

Better evidence than the word comes from the delivered accounts. After a small test order, open a sample of them and check whether they have posts, whether their follower and following numbers look plausible, and whether they share names, photos or creation patterns. Check too whether the service has a refill window. A small test order costs less than finding out on a large one.

What it does not mean

Real does not mean the engagement will stay, will interact with future posts, or will be treated as authentic by the platform. Genuine-looking accounts can still be removed, and accounts run by people can still unfollow. It does not mean the rate reflects the cost of real people either: labels and prices are set independently.
